Kent to award grants to organizations helping disparate populations – Conduit Street

The Kent County Health Department will assign 25 scholarships up to $15,000 to organizations that will provide services or education to disparate populations in Kent County.

The network established through the grant award will be used to increase the capacity to reach disproportionately affected populations with effective cultural and linguistic programs and practices. This will include health and non-health interventions which may include access to health care, social services, transport and housing.

The Kent County Health Department (KHCD) seeks to address disparities between high-risk populations and underserved communities through this grant. These communities include, but are not limited to, rural residents, elderly residents (65+), Hispanic residents, African American residents, homeless residents, and residents with mental illness. KCHD intends to use a team of nursing and administrative coordinators in addition to translation services to focus on the following 3 activities: 1. Forming community partnerships, 2. Mobilizing partners and collaborators, and 3. Expand existing resources/develop new mitigation and prevention resources and services. KCHD intends to award grants to partner organizations to help rapidly mobilize selected mitigation efforts in Kent County postcodes.
